Arizona Long Term Care System (ALTCS) Program

ALTCS, tribal case management, is an integrated health plan. The partnership is formed by an Intergovernmental Agreement between Arizona Health Care Cost Containment System (AHCCCS) & the San Carlos Apache Tribe (SCAT) to provide long-term care case management services to tribal members enrolled in ALTCS. ALTCS elderly, physically disabled long-term plan is for individuals of all ages who need ongoing care. Eligibility criteria are based on medical and financial information.

As an ALTCS member, you are eligible to receive the listed services including: 

  • Attendant Care, in-home personal care attendant
  • Durable Medical Equipment/Medical Supplies
  • Emergency Alert System
  • Home Delivered Meals
  • Home Modification, creating a handicapped accessible home
  • Hospice Services (Community and Institutional)
  • Skilled Nursing Facility Services
  • Assisted Living services
  • Therapies; Physical, Occupational, Speech.
  • Non-Emergency Medical Transportation to medical appointments

Filing an Application for ALTCS

ALTCS is the State of Arizona’s Medicaid program that provides long term care services, at little or no cost, to financially and medically eligible Arizona residents who are aged, blind, disabled, or have a developmental disability.

You must also meet medical eligibility requirements. More About ALTCS

As a Tribal ALTCS member you will have your own case manager, choose the type of services you need. You have help and caring people on your side. Case Managers coordinate care, meaning assisting with medical supplies or working with an agency to get in home care provider services. They will visit you to check how you are doing, ask questions about your goals and what health services you need. If you have a care provider, it could be a family member or health care professional. You can choose where you live, home, assisted living home or a skilled nursing home. The goal for members is to be the healthiest & happiest you can be while still enjoying much independence as possible.

You can apply through AHCCCS. To apply for Long Term Care, call or visit the local ALTCS office. You can also download an application from the AHCCCS website.
