Community Health Representatives (CHR) Program

The CHR Program provides health care, health education, wellness activities and group/individual counseling to all communities on the San Carlos Apache Reservation.

Our staff of well-trained professionals delivers health care services in various settings i.e. home, office, hospitals, and community health events.

 

Establishment/Purpose:

After a survey of health issues of Native Americans, it was found that many of the health problems of our tribal members required a program or services especially adapted to Tribal situations. The survey also found that services should be provided by Tribal members, the reason being is that they are familiar with customs, traditions and above all – know the Apache language. To meet that need the CHR Program was established in 1968. CHR has become an important liaison between the San Carlos Apache Community and the San Carlos Apache Health care.

Eligibility and Available Services

All individuals residing within the boundaries of the San Carlos Apache Reservation are eligible.

    • Providing health education, prevention education to the Community
  • Case/Find Screenings
    • Vital checks
    • Assist with home safety survey
    • Blood pressure checks
    • Head lice checks
    • Assist with Well Baby
    • Assist with school physicals
  • Case Management
    • Medication delivery
    • Transportation
    • Advocating/Translation
  • Monitor Patients
    • Home visits
  • Patient Care
    • Foot care
    • Emotional support
    • Provide personal care
    • Vital Sign checks
  • Homemaker Services for Elders/Disabled
    • Grocery shopping, washing dishes, cleaning home, food preparation, lawn care
  • Transportation
    • CHR provides non-emergency medical transportation within the San Carlos Apache communities as well as off-reservation.

HEALTH START 

The Health Start Program is a community outreach initiative under CHR that offers free assistance to expecting mothers, new mothers, and their families. The program focuses on education topics and connects families with local resources. Topics covered include prenatal care, parenting, breastfeeding, well childcare, and home safety. CHR can provide a free car seat and provide free car seat installation.

  • FREE diapers (newborn to size 5)
  • Monthly incentives
  • Pack-N-Play
  • Infant car seats (while supplies last)
