Public Health Nursing (PHN) Program

The scope of work for our PHN program is population based and focuses on a collection of individuals who have one or more personal or environmental characteristics in common. PHNs promote health and disease prevention in the San Carlos Apache community. Our practice involves populations possessing similar health concerns and characteristics. 

Services:

  • Provide education, resources and treatment for Communicable Diseases
  • Provide education and resources for high-risk patients in the community
  • Participate in community screenings and education

Our PHNs serve the community by doing home visits, providing treatment for infectious disease, providing education, phone call follow- ups, chart review and helping at community events.

Under PHN, San Carlos Apache Tribe (SCAT) Department of Health & Human Services (DHHS) also has a Community Nutrition Program and Health Education.

Community Nutrition

Community Nutrition develops menus for the community programs such as the Older Adult Center, Elder Care program, Tribal Youth Home and other programs in the community that request assistance.

Community Nutrition also provides nutrition therapy for individuals referred by Women, Infant and Children’s program, the Maternal Child Health clinic and Public Health Nursing.

The program also provides nutrition education sessions covering various topics: child nutrition and healthy eating habits as requested by local schools.

Community Nutrition is actively involved within the San Carlos community, often partnering with other SCAT DHHS programs at health fairs and community events.

 

The Health Education program promotes wellness, disease prevention, and healthy lifestyle choices within the community.  The education department provides educational activities at community events, school functions and health fairs in the San Carlos Community.
